The Community Liaison is responsible for cultivating relationships with physicians, clinic and physician staff, hospital Case Managers and the community. This full time position blends both the skills of the selected candidate with their sales and marketing attributes.
Working on behalf of Senior Solutions At Home, Inc. this position will be responsible for maintaining and growing referrals for both Care Management and In-Home Care Services. The Liaison is accountable for proactively impacting Agency growth and referral source volume and satisfaction.
Full-time position with base plus incentive plan.

Participate in care team conferences to ensure client satisfaction and continuity of services delivered
Assist with the implementation and development of programs, and produce required reports, summaries and recommendations
Use skills in valuable client interaction
Use superior customer service and sales knowledge to attract and maintain business relationships in the health care industry
Establish, coordinate, and maintain all community relations functions
Coordinate the referral process from the identification of the appropriate payer source to the analysis of patient information
Use business skills to build a strategy for identifying and growing new business

| Qualifications: | Degree in business and/or management preferred.
Sales and marketing experience strongly preferred
Self-directed with the ability to work with little supervision.
Two years experience in the health care industry preferred
Competent organizational skills
Ability to handle challenging situations/deadlines
Excellent oral/written communication and
interpersonal skills
Must demonstrate the ability to communicate effectively with all members of the health care team
Demonstrate the ability to support the philosophy, objectives and goals of the agency.
Forecasts needs; sets priorities
Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars
Ability to self-manage online HR services and online training programs. |
