| Description: | Assist individuals with daily living activities at the person's home. Duties performed at a place of residence may include keeping house (making beds, doing laundry, washing dishes) and preparing meals. Transport Client to medical appointments.
Will work under the supervision of a Field Supervisor.

| Duties: | Prepare and maintain records of client progress and services performed per company policy.
Perform housekeeping duties, such as cooking, cleaning, washing clothes and dishes.
Personal Care assistance.
Lifting and transferring of patient.
Perform other duties as necessary.
Follow policies and procedures related to safety.
Conduct visual safety inspections throughout the work day; identify and report unsafe conditions and correct as needed.
Immediately report incidents/injuries to supervisor.
Incorporate safe behaviors into daily work life.
Demonstrate appropriate body mechanics in all situations.

| Qualifications: | High School Diploma or GED.
CNT certification preferred
Six months to one year related experience.
State Licensed Driver
CPR/First Aide Certified.
Curent T.B skin test |
